objtool: Refactor ORC section generation

commit ab4e0744e99b87e1a223e89fc3c9ae44f727c9a6 upstream.

Decouple ORC entries from instructions.  This simplifies the
control/data flow, and is going to make it easier to support alternative
instructions which change the stack layout.
