This patch adds support for the realtek 8211c phy. The driver must
perform a hardware reset of the phy due to an errata where the phy could
not detect the link.

This patch fixes mac80211 to not use the skb->cb over the queue step
from virtual interfaces to the master. The patch also, for now,
disables aggregation because that would still require requeuing,
will fix that in a separate patch. There are two other places (software
requeue and powersaving stations) where requeue can happen, but that is
not currently used by any drivers/not possible to use respectively.

Wireless statistics produced by the RTL8187B driver are not particularly
informative about the strength of the received signal. From the data sheet
provided by Realtek, I discovered that certain parts of the RX header
should have the information necessary to calculate signal quality and
strength. With testing, it became clear that most of these quantities were
very jittery - only the AGC correlated with the signals expected from nearby
AP's. As a result, the quality and strength are derived from the agc value.
The scaling has been determined so that the numbers are close to those
obtained by b43 under the same conditions. The results are qualitatively
correct.
Statistics derived for the RTL8187 have not been changed.
The RX header variables have been renamed to match the quantites described
in the Realtek data sheet.

"mac80211: fix TX sequence numbers" broke rtl8187. This
patch makes the same kind of fix that was done for rt2x00. Note that
this code will have to be reworked for proper sequence numbers on beacons.
In addition, the sequence number has been placed in the hardware state,
not the vif state.

When the queues are being initialized the entry flags fields must be
reset to 0. When this does not happen some entries might still be
marked as "occupied" after an ifdown & ifup cycle which would trigger
errors when the entry is being accessed:
phy0 -> rt2x00queue_write_tx_frame: Error - Arrived at non-free entry in the non-full queue 0.

rt2x00 will only perform configuration changes from
mac80211 when the configuration option has changed.
This means it keeps track of the current active configuration
and will check these values when the config() callback function
is used.
However this causes breakage when the interface has been
brought down and up again, since all stored active values
aren't reset while the registers might have.
This is for example the case with rt61pci antenna registers which
will jump to invalid values when the interface has been started.
To make sure a full configuration takes place after the start()
callback function, a new flag is added which will be checked
during config() and skips the "what's changed" phase.

When IEEE80211_TX_CTL_ASSIGN_SEQ is not set,
the driver should disable hardware sequence counting
to make sure the mac80211 provided counter is used.
This fixes QOS sequence counting, since that is one
of the cases where mac80211 provides a seperate
sequence counter.
By moving the sequence counting code to rt2x00queue
we make sure that _all_ frames get the sequence counter,
including RTS/CTS and Beacon frames.

When setting up a beacon template, the length of the beacon is
calculated with the assumption that the SKB already contains
the Tx descriptor. In the case of beacons it doesn't.
This patch undoes the damage by adding the Tx descriptor length
to the beacon length. This is safe, because the shortest possible
beacon is longer than the Tx header.

Adds an extra rt2x00 vendor request function to support register
transfers beyond the CSR_CACHE_SIZE / USB packet size limit. This
is useful for firmware uploads, beacon templates and keys, all
of which are to large to do with a single USB request.

Olivier reported a difference between the EIFS
values used in the legacy driver and the one in
the rt2x00 drivers.
In rt2x00 the value was
( SIFS + (8 * (IEEE80211_HEADER + ACK_SIZE)) )
which comes down to 314us while the legacy driver uses the value 364us
This was caused because EIFS is: SIFS + DIFS + AckTime
This patch will fix this by adding the DIFS by the above value,
and creating a SHORT_EIFS define which uses the SHORT_DIFS.

Make sure that the irq is not in progress after stop. This means
two things:
- ensure the intr setting register is set by flushing posted values
- call synchronize_irq() after that
Also flush stop tx write, inform callers of the tx stop about still
pending transfers (unsuccessful stop) and finally don't wait another
3ms in ath5k_rx_stop, since ath5k_hw_stop_rx_dma ensures transfer to
be finished.
Make sure all writes will be ordered in respect to locks by mmiowb().

When signal is noisy, hardware can use all RX buffers and since the last
entry in the list is self-linked, it overwrites the entry until we link
new buffers.
Ensure that we don't free this last one until we are 100% sure that it
is not used by the hardware anymore to not cause memory curruption as
can be seen below.
This is done by checking next buffer in the list. Even after that we
know that the hardware refetched the new link and proceeded further
(the next buffer is ready) we can finally free the overwritten buffer.
We discard it since the status in its descriptor is overwritten (OR-ed
by new status) too.

If we can be sure that elevating the page_count on a pagecache page will
pin it, we can speculatively run this operation, and subsequently check to
see if we hit the right page rather than relying on holding a lock or
otherwise pinning a reference to the page.
This can be done if get_page/put_page behaves consistently throughout the
whole tree (ie. if we "get" the page after it has been used for something
else, we must be able to free it with a put_page).
Actually, there is a period where the count behaves differently: when the
page is free or if it is a constituent page of a compound page. We need
an atomic_inc_not_zero operation to ensure we don't try to grab the page
in either case.
This patch introduces the core locking protocol to the pagecache (ie.
adds page_cache_get_speculative, and tweaks some update-side code to make
it work).
Thanks to Hugh for pointing out an improvement to the algorithm setting
page_count to zero when we have control of all references, in order to
hold off speculative getters.

Add per-device dma_mapping_ops support for CONFIG_X86_64 as POWER
architecture does:
This enables us to cleanly fix the Calgary IOMMU issue that some devices
are not behind the IOMMU (http://lkml.org/lkml/2008/5/8/423).
I think that per-device dma_mapping_ops support would be also helpful for
KVM people to support PCI passthrough but Andi thinks that this makes it
difficult to support the PCI passthrough (see the above thread). So I
CC'ed this to KVM camp. Comments are appreciated.
A pointer to dma_mapping_ops to struct dev_archdata is added. If the
pointer is non NULL, DMA operations in asm/dma-mapping.h use it. If it's
NULL, the system-wide dma_ops pointer is used as before.
If it's useful for KVM people, I plan to implement a mechanism to register
a hook called when a new pci (or dma capable) device is created (it works
with hot plugging). It enables IOMMUs to set up an appropriate
dma_mapping_ops per device.
The major obstacle is that dma_mapping_error doesn't take a pointer to the
device unlike other DMA operations. So x86 can't have dma_mapping_ops per
device. Note all the POWER IOMMUs use the same dma_mapping_error function
so this is not a problem for POWER but x86 IOMMUs use different
dma_mapping_error functions.
The first patch adds the device argument to dma_mapping_error. The patch
is trivial but large since it touches lots of drivers and dma-mapping.h in
all the architecture.
This patch:
dma_mapping_error() doesn't take a pointer to the device unlike other DMA
operations. So we can't have dma_mapping_ops per device.
Note that POWER already has dma_mapping_ops per device but all the POWER
IOMMUs use the same dma_mapping_error function. x86 IOMMUs use device
argument.

Removes legacy reinvent-the-wheel type thing. The generic
machinery integrates much better to automated debugging aids
such as kerneloops.org (and others), and is unambiguous due to
better naming. Non-intuively BUG_TRAP() is actually equal to
WARN_ON() rather than BUG_ON().

Enable ibmveth for Cooperative Memory Overcommitment (CMO). For this driver
it means calculating a desired amount of IO memory based on the current MTU
and updating this value with the bus when MTU changes occur. Because DMA
mappings can fail, we have added a bounce buffer for temporary cases where
the driver can not map IO memory for the buffer pool.
The following changes are made to enable the driver for CMO:
* DMA mapping errors will not result in error messages if entitlement has
been exceeded and resources were not available.
* DMA mapping errors are handled gracefully, ibmveth_replenish_buffer_pool()
is corrected to check the return from dma_map_single and fail gracefully.
* The driver will have a get_desired_dma function defined to function
in a CMO environment.
* When the MTU is changed, the driver will update the device IO entitlement

If we hack the virtio_net driver to always allocate full-sized (64k+)
skbuffs, the driver slows down (lguest numbers):
Time to receive 1GB (small buffers): 10.85 seconds
Time to receive 1GB (64k+ buffers): 24.75 seconds
Of course, large buffers use up more space in the ring, so we increase
that from 128 to 2048:
Time to receive 1GB (64k+ buffers, 2k ring): 16.61 seconds
If we recycle pages rather than using alloc_page/free_page:
Time to receive 1GB (64k+ buffers, 2k ring, recycle pages): 10.81 seconds
This demonstrates that with efficient allocation, we don't need to
have a separate "small buffer" queue.

Finally this patch lets virtio_net receive GSO packets in addition
to sending them. This can definitely be optimised for the non-GSO
case. For comparison the Xen approach stores one page in each skb
and uses subsequent skb's pages to construct an SG skb instead of
preallocating the maximum amount of pages per skb.

On 32-bit architectures PAGE_ALIGN() truncates 64-bit values to the 32-bit
boundary. For example:
u64 val = PAGE_ALIGN(size);
always returns a value < 4GB even if size is greater than 4GB.
The problem resides in PAGE_MASK definition (from include/asm-x86/page.h for
example):

The "~" is performed on a 32-bit value, so everything in "and" with
PAGE_MASK greater than 4GB will be truncated to the 32-bit boundary.
Using the ALIGN() macro seems to be the right way, because it uses
typeof(addr) for the mask.
Also move the PAGE_ALIGN() definitions out of include/asm-*/page.h in
include/linux/mm.h.

Although mv643xx_eth has no hardware support for inserting a vlan
tag by twiddling some bits in the TX descriptor, it does support
hardware TX checksumming on packets where the IP header starts {a
limited set of values other than 14} bytes into the packet.
This patch sets mv643xx_eth's ->vlan_features to NETIF_F_SG |
NETIF_F_IP_CSUM, which prevents the stack from checksumming vlan'ed
packets in software, and if vlan tags are present on a transmitted
packet, notifies the hardware of this fact by toggling the right
bits in the TX descriptor.

The mv643xx_eth hardware has a provision for polling the PHY's
MII management registers to obtain the (R)(G)MII interface speed
(10/100/1000) and duplex (half/full) and pause (off/symmetric)
settings to use to talk to the PHY.
The driver currently does not make use of this feature. Instead,
whenever there is a link status change event, it reads the current
link parameters from the PHY, and programs those parameters into
the mv643xx_eth MAC by hand.
This patch switches the mv643xx_eth driver to letting the MAC
auto-determine the (R)(G)MII link parameters by PHY polling, if there
is a PHY present. For PHYless ports (when e.g. the (R)(G)MII
interface is connected to a hardware switch), we keep hardcoding the
MII interface parameters.

The mv643xx_eth driver is limiting DMA bursts to 32 bytes, while
using the largest burst size (128 bytes) gives a couple percentage
points performance improvement in throughput tests, and the docs
say that the 128 byte default should not need to be changed, so
use 128 byte bursts instead.

The recommended sequence for waiting for the transmit path to clear
after disabling all of the transmit queues is to wait for the
TX_FIFO_EMPTY bit in the Port Status register to become set as well
as the TX_IN_PROGRESS bit to clear.

The maximum receive packet size field in the Port Serial Control
register controls at what size received packets are flagged
overlength in the receive descriptor, but it doesn't prevent
overlength packets from being DMAd to memory and signaled to the
host like other received packets.
mv643xx_eth does not support receiving jumbo frames in 10/100 mode,
but setting the packet threshold to larger than 1522 bytes in 10/100
mode won't cause breakage by itself.
If we really want to enforce maximum packet size on the receiving
end instead of on the sending end where it should be done, we can
always just add a length check to the software receive handler
instead of relying on the hardware to do the comparison for us.
What's more, changing the maximum packet size field requires
temporarily disabling the RX/TX paths. So once the link comes
up in 10/100 Mb/s mode or 1000 Mb/s mode, we'd have to disable it
again just to set the right maximum packet size field (1522 in
10/100 Mb/s mode or 9700 in 1000 Mb/s mode), just so that we can
offload one comparison operation to hardware that we might as well
do in software, assuming that we'd want to do it at all.
Contrary to what the documentation suggests, there is no harm in
just setting a 9700 byte maximum packet size in 10/100 mode, so use
the maximum maximum packet size for all modes.

The mv643xx_eth driver allows doing transmit reclaim from within the
napi poll routine, but after doing reclaim, it would forget to check
the free transmit descriptor count and wake up the transmit queue if
the reclaim caused enough descriptors for a new packet to become
available. This would cause the netdev watchdog to occasionally kick
in during certain workloads with combined receive and transmit traffic.
Fix this by adding a wakeup check identical to the one in the
interrupt handler to the napi poll routine.

When the ethernet link goes down while mv643xx_eth is transmitting
data, transmit DMA can stop before all queued transmit descriptors
have been processed. But even the descriptors that _have_ been
processed might not be properly marked as done before the transmit
DMA unit shuts down.
Then when the link comes up again, the hardware transmit pointer
might have advanced while not all previous packet descriptors have
been marked as transmitted, causing software transmit reclaim to
hang waiting for the hardware to finish transmitting a descriptor
that it has already skipped.
This patch forcibly reclaims all packets on the transmit ring on a
link down interrupt, and then resyncs the hardware transmit pointer to
what the software's idea of the first free descriptor is. Also, we
need to prevent re-waking the transmit queue if we get a 'transmit
done' interrupt at the same time as a 'link down' interrupt, which
this patch does as well.

The previously merged TX hang erratum workaround ("mv643xx_eth:
work around TX hang hardware issue") assumes that TX_END interrupts
are delivered simultaneously with or after their corresponding TX
interrupts, but this is not always true in practise.
In particular, it appears that TX_END interrupts are issued as soon
as descriptor fetch returns an invalid descriptor, which may happen
before earlier descriptors have been fully transmitted and written
back to memory as being done.
This hardware behavior can lead to a situation where the current
driver code mistakenly assumes that the MAC has given up transmitting
before noticing the packets that it is in fact still currently working
on, causing the driver to re-kick the transmit queue, which will only
cause the MAC to re-fetch the invalid head descriptor, and generate
another TX_END interrupt, et cetera, until the packets in the pipe
finally finish transmitting and have their descriptors written back
to memory, which will then finally break the loop.
Fix this by having the erratum workaround not check the 'number of
unfinished descriptor', but instead, to compare the software's idea
of what the head descriptor pointer should be to the hardware's head
descriptor pointer (which is updated on the same conditions as the
TX_END interupt is generated on, i.e. possibly before all previous
descriptors have been transmitted and written back).

Evgeniy Polyakov noticed that drivers/net/e1000e/netdev.c:e1000_netpoll()
was calling e1000_clean_tx_irq() without taking the TX lock.
David Miller suggested to remove the call altogether: since in this
callpah there's periodic calls to ->poll() anyway which will do
e1000_clean_tx_irq() and will garbage-collect any finished TX ring
descriptors.
This fix solved the e1000e+netconsole crashes i've been seeing:

If an mlx4 device with default FW (which gives a UAR BAR size of 8 MB)
is used in a system with 64 KB pages, then there are only 8192/64==128
UAR pages available. However, the first 128 UAR pages are reserved
for use with event queue doorbells, so no UAR pages are available to
do anything else with, which means that the driver cannot work.
The current driver fails with a fairly cryptic "Failed to allocate
driver access region, aborting" message in this situation. Fix the
driver to detect the problem earlier and print out a clearer
description of the problem and a suggestion of how to fix it (use a
new firmware image).

netif_carrier_{on,off}() handles starting and stopping packet
flow into the driver. So there is no reason to invoke netif_stop_queue()

This patch bumps the release of the r6040 driver. There has been
quite some versions of it out there, but this one is the one
people should report bugs against.

This patch changes the default waiting time of a packet, which
along with our previous r6040_rx path, was causing huge delays

This patch completely reworks the RX path in order to be
more accurate about what is going on with the MAC.
We no longer read the error from the MLSR register instead read
the descriptor status register which reflects, the error per descriptor.
We now allocate skbs on the fly in r6040_rx, and we handle allocation
failure instead of simply dropping the packet. Remove the
rx_free_desc counter of r6040_private structure since we
allocate skbs in the RX path.
r6040_rx_buf_alloc is now removed and becomes unuseless.

("mv643xx_eth: clarify irq masking and unmasking") broke the
mv643xx_eth build when NETPOLL is enabled, due to it not renaming

Rework the RX buffers allocation function so that we do not
leak memory in the case we could not allocate skbs for the
RX path. Propagate the errors to the r6040_up function
where we call the RX buffers allocation function.
Also rename the r6040_alloc_txbufs function to
r6040_init_txbufs, to reflect what it really does.

Add a helper function which only modifies R6040 MAC registers
use it when we timeout, and on adapter initialization. Fix
the scheduling while atomic but in the timeout routine due
to the reallocation of rx/tx buffers.

Using multiple cores in the same package to handle received traffic
does not appear to provide a performance benefit. Therefore use CPU
topology information to count CPU packages and use that as the default
number of RX queues and interrupts. We rely on interrupt balancing to
spread the interrupts across packages.

This patch makes e1000 driver ioport-free.
This corrects behavior in probe function so as not to request ioport
resources as long as they are not really needed. This is based on the
ioport-free patch of e1000 driver from Auke Kok and Tomohiro Kusumi.

The ixgbe driver was untested with device ID 8086:10c8 but still advertises
support. Currently if this device is present in the system when the driver
is loaded, the system will panic.
Remove this device ID until full support can be tested with available
hardware. This patch is necessary for 2.6.24, 2.6.25 and 2.6.26

The drivers were touching net queue before it has been started, so
without this patch, the drivers will potentially WARN at
net/core/dev.c:1328.
I don't have the hardware for the drivers below, so this patch is
untested, and thus should be carefully peer reviewed.

The OQO model 2 has an RTL8139 from Atheros that doesn't like MMIO.
It fails on modprobe, with a 'PCI Bus error' message.

This is needed to support devices that only work with PIO without
penalising devices that work fine with MMIO in distro kernels.
It also allows us to eventually use PIO as a fallback when setting
up MMIO fails.

NX3031 supports cut-through operation where ingress packets are
directly dma'ed into host buffers to reduce latency.
This requires larger dma buffers (2kb) and different alignemnt.
The buffer posting logic is changed a bit. The free rx buffers
are maintained in linked list, since the received reference
handles can be out of order. However rx descriptors are still
posted sequentially, indexed by producer.

MAC addr, multicast filters, mtu are set through firmware commands
in firmware v4.0.0+ because of virtualization of physical ports.
Link status is also read from registers allocated by firmware for
each virtual port.

MTT entries are allocated with a buddy allocator, which just keeps
bitmaps for each level of the buddy table. However, all free space
starts out at the highest order, and small allocations start scanning
from the lowest order. When the lowest order tables have no free
space, this can lead to scanning potentially millions of bits before
finding a free entry at a higher order.
We can avoid this by just keeping a count of how many free entries
each order has, and skipping the bitmap scan when an order is
completely empty. This provides a nice performance boost for a
negligible increase in memory usage.

seems closest.
This is in preparation for providing more detailed log info using
mlx4_err() in low-level driver when a non-zero status is returned.

Add a module parameter "enable_qos" to mlx4_core. If this param is
set, enable support for QoS in the INIT_HCA command. By default, the
parameter is set to 0 (disabled).

Switch to ioremap_wc(). We keep the MTRR code since ioremap_wc()
will use UC_MINUS when falling back to uncachable, and thus let
the MTRR WC take precedence.
Also rename the error path better.

IFF_PROMISC flag shouldn't be set or cleared by drivers, because
whether device be promisc mode is decided by how many upper layer
callers being referenced to it.
And the promisc changing feature of de4x5 ioctl is developer debug
feature, we can remove it now.

I was looking at this out of interest, but I'm in no way familiar with
the code.
Looks to me that the error handling code in mac80211_hwsim is awkward.
Which leads to it calling ieee80211_unregister_hw even when
ieee80211_register_hw failed.
The function has a for loop where it generates all simulated radios.
when something fails, the error handling will call mac80211_hwsim_free
which frees all simulated radios who's pointer isn't zero. However the
information stored is insufficient to determine whether or not the call
to ieee80211_register_hw succeeded or not for a specific radio. The
included patch makes init_mac80211_hwsim clean up the current simulated
radio, and then calls into mac80211_hwsim_free to clean up all the
radios that did succeed.
This however doesn't explain why the rate control registration failed..
build tested this, but had some problems reproducing the original
problem.

This is a trivial patch against the hdlcdrv module that fixes its CRC
calculation. The finished CRC was overwriting the first two bytes of
each packet rather than being appended to the end.
I've tested this with 2.6.8 and 2.6.10-rc1, but hdlcdrv hasn't changed
much recently so it should work with many other kernel versions.

Based upon a bug report by Alexey Dobriyan, the patch is
also tested by him and confirmed to fix the problem.
Packet flow during link state events should not be done by
waking and stopping the TX queue anyways, that is handled
transparently by netif_carrier_{on,off}().
So, remove the netif_{wake,stop}_queue() calls in the link
check code, and add the necessary netif_start_queue() call
to atl1_up().
