Jeremy Fitzhardinge ecbf29cdb3 xen: clean up asm/xen/hypervisor.h
Impact: cleanup

hypervisor.h had accumulated a lot of crud, including lots of spurious
#includes.  Clean it all up, and go around fixing up everything else
accordingly.
