35589a8fa8 [IA64] Move perfmon tables from thread_struct to pfm_context
This patch renders thread_struct->pmcs[] and thread_struct->pmds[]
OBSOLETE. The actual table is moved to pfm_context structure which
saves space in thread_struct (in turn saving space in task_struct
which frees up more space for kernel stacks).

214ddde2f9 [IA64] kprobe opcode 16 bytes alignment on IA64
On IA64 instruction opcode must be 16 bytes alignment, in kprobe structure
there is one element to save original instruction, currently saved opcode
is not statically allocated in kprobe structure, that can not assure
16 bytes alignment. This patch dynamically allocated kprobe instruction
opcode to assure 16 bytes alignment.

46a82b2d55 [PATCH] Standardize pxx_page macros
One of the changes necessary for shared page tables is to standardize the
pxx_page macros.  pte_page and pmd_page have always returned the struct
page associated with their entry, while pte_page_kernel and pmd_page_kernel
have returned the kernel virtual address.  pud_page and pgd_page, on the
other hand, return the kernel virtual address.

Shared page tables needs pud_page and pgd_page to return the actual page
structures.  There are very few actual users of these functions, so it is
simple to standardize their usage.

Since this is basic cleanup, I am submitting these changes as a standalone
patch.  Per Hugh Dickins' comments about it, I am also changing the
pxx_page_kernel macros to pxx_page_vaddr to clarify their meaning.

3212fe1594 [PATCH] cpu to node relationship fixup: map cpu to node
Assume that a cpu is *physically* offlined at boot time...

Because smpboot.c::smp_boot_cpu_map() canoot find cpu's sapicid,
numa.c::build_cpu_to_node_map() cannot build cpu<->node map for
offlined cpu.

For such cpus, cpu_to_node map should be fixed at cpu-hot-add.
This mapping should be done before cpu onlining.

This patch also handles cpu hotremove case.

3a45975681 [PATCH] IA64,sparc: local DoS with corrupted ELFs
This prevents cross-region mappings on IA64 and SPARC which could lead
to system crash.  They were correctly trapped for normal mmap() calls,
but not for the kernel internal calls generated by executable loading.

This code just moves the architecture-specific cross-region checks into
an arch-specific "arch_mmap_check()" macro, and defines that for the
architectures that needed it (ia64, sparc and sparc64).

Architectures that don't have any special requirements can just ignore
the new cross-region check, since the mmap() code will just notice on
its own when the macro isn't defined.

986e12fa74 [IA64] remove redundant local_irq_save() calls from sn_sal.h
sn_change_memprotect() does a local_irq_save() then calls
ia64_sal_oemcall_nolock() which calls SAL_CALL_NOLOCK()
which also does a local_irq_save().

This patch removes the redundant local_irq_save() and local_irq_restore()
calls in sn_change_memprotect() and sn_inject_error().

Allow per platform setting of IDE legacy bases

Some of this can be tidied further later on, in particular all the
legacy port gunge belongs as a PCI quirk/PCI header decode to understand
the special legacy IDE rules in the PCI spec.

Longer term Jeff also wants to move the request_irq/free_irq out of core
which will make this even cleaner.

tj: folded in three followup patches - ata_piix-fix, broken-arch-fix
and fix-new-legacy-handling, and separated per-dev xfermask into
separate patch preceding this one.  Folded in fixes are...

* ata_piix-fix: fix build failure due to host_set->next removal
* broken-arch-fix: add missing include/asm-*/libata-portmap.h
* fix-new-legacy-handling:
	* In ata_pci_init_legacy_port(), probe_num was incorrectly
          incremented during initialization of the secondary port and
          probe_ent->n_ports was incorrectly fixed to 1.

	* Both legacy ports ended up having the same hard_port_no.

	* When printing port information, both legacy ports printed
	  the first irq.

7682a4c624 [IA64-SGI] Silent data corruption caused by XPC V2.
Jack Steiner identified a problem where XPC can cause a silent
data corruption.  On module load, the placement may cause the
xpc_remote_copy_buffer to span two physical pages.  DMA transfers are
done to the start virtual address translated to physical.

This patch changes the buffer from a statically allocated buffer to a
kmalloc'd buffer.  Dean Nelson reviewed this before posting.  I have
tested it in the configuration that was showing the memory corruption
and verified it works.  I also added a BUG_ON statement to help catch
this if a similar situation is encountered.

e44e41d0c8 [IA64] fix show_mem for VIRTUAL_MEM_MAP+FLATMEM
contig.c (FLATMEM) requires the same optimization as in discontig.c for show_mem
when VIRTUAL_MEM_MAP is in use. Otherwise FLATMEM has softlockup timeouts.
This was boot tested for memory configuration: SPARSEMEM,
DISCONTIG+VIRTUAL_MEM_MAP, FLATMEM, FLATMEM+VIRTUAL_MEM_MAP and
FLATMEM+VIRTUAL_MEM_MAP with largest memory gap less than LARGE_GAP by
using boot parameter "mem=".

This was boot tested and "echo m >/proc/sysrq-trigger" output evaluated for
: FLATMEM, FLATMEM+VIRTUAL_MEM_MAP, DISCONTIGMEM+VIRTUAL_MEM_MAP and
SPARSEMEM.

a9ad965ea9 [PATCH] IA64: kprobe invalidate icache of jump buffer
Kprobe inserts breakpoint instruction in probepoint and then jumps to
instruction slot when breakpoint is hit, the instruction slot icache must
be consistent with dcache.  Here is the patch which invalidates instruction
slot icache area.

Without this patch, in some machines there will be fault when executing
instruction slot where icache content is inconsistent with dcache.

1bf1eba74e [IA64] Format /proc/pal/*/version_info correctly
/proc/pal/*/version_info is a bit confusing.  HP firmware, at least,
reports 07.31 instead of 0.7.31.  Also, the comment is out of place;
it's an internal detail about the implementation of ia64_pal_version.
Since the 2.2 revision of the SDM still states that PAL_VERSION can
be called in virtual mode, correct the comment to be more accurate.

52393ccc0a [PATCH] remove set_wmb - arch removal
set_wmb should not be used in the kernel because it just confuses the
code more and has no benefit.  Since it is not currently used in the
kernel this patch removes it so that new code does not include it.

All archs define set_wmb(var, value) to do { var = value; wmb(); }
while(0) except ia64 and sparc which use a mb() instead.  But this is
still moot since it is not used anyway.

Hasn't been tested on any archs but x86 and x86_64 (and only compiled
tested)

06c67befee [PATCH] make valid_mmap_phys_addr_range() take a pfn
Newer ARMs have a 40 bit physical address space, but mapping physical
memory above 4G needs a special page table format which we (currently?) do
not use for userspace mappings, so what happens instead is that mapping an
address >= 4G will happily discard the upper bits and wrap.

There is a valid_mmap_phys_addr_range() arch hook where we could check for
>= 4G addresses and deny the mapping, but this hook takes an unsigned long
address:

	static inline int valid_mmap_phys_addr_range(unsigned long addr, size_t size);

And drivers/char/mem.c:mmap_mem() calls it like this:

So that's not much help either.

36c8b58689 [PATCH] sched: cleanup, remove task_t, convert to struct task_struct
cleanup: remove task_t and convert all the uses to struct task_struct. I
introduced it for the scheduler anno and it was a mistake.

Conversion was mostly scripted, the result was reviewed and all
secondary whitespace and style impact (if any) was fixed up by hand.

877ce7c1b3 [AF_UNIX]: Datagram getpeersec
This patch implements an API whereby an application can determine the
label of its peer's Unix datagram sockets via the auxiliary data mechanism of
recvmsg.

Patch purpose:

This patch enables a security-aware application to retrieve the
security context of the peer of a Unix datagram socket.  The application
can then use this security context to determine the security context for
processing on behalf of the peer who sent the packet.

Patch design and implementation:

The design and implementation is very similar to the UDP case for INET
sockets.  Basically we build upon the existing Unix domain socket API for
retrieving user credentials.  Linux offers the API for obtaining user
credentials via ancillary messages (i.e., out of band/control messages
that are bundled together with a normal message).  To retrieve the security
context, the application first indicates to the kernel such desire by
setting the SO_PASSSEC option via getsockopt.  Then the application
retrieves the security context using the auxiliary data mechanism.

An example server application for Unix datagram socket should look like this:

sock_setsockopt is enhanced with a new socket option SOCK_PASSSEC to allow
a server socket to receive security context of the peer.

Testing:

We have tested the patch by setting up Unix datagram client and server
applications.  We verified that the server can retrieve the security context
using the auxiliary data mechanism of recvmsg.

5c45bf279d [PATCH] sched: mc/smt power savings sched policy
sysfs entries 'sched_mc_power_savings' and 'sched_smt_power_savings' in
/sys/devices/system/cpu/ control the MC/SMT power savings policy for the
scheduler.

Based on the values (1-enable, 0-disable) for these controls, sched groups
cpu power will be determined for different domains.  When power savings
policy is enabled and under light load conditions, scheduler will minimize
the physical packages/cpu cores carrying the load and thus conserving
power(with a perf impact based on the workload characteristics...  see OLS
2005 CMP kernel scheduler paper for more details..)

7049027c6f [PATCH] pgdat allocation and update for ia64 of memory hotplug: update pgdat address array
This is to refresh node_data[] array for ia64.  As I mentioned previous
patches, ia64 has copies of information of pgdat address array on each node as
per node data.

At v2 of node_add, this function used stop_machine_run() to update them.  (I
wished that they were copied safety as much as possible.) But, in this patch,
this arrays are just copied simply, and set node_online_map bit after
completion of pgdat initialization.

So, kernel must touch NODE_DATA() macro after checking node_online_map().
(Current code has already done it.) This is more simple way for just
hot-add.....

Note : It will be problem when hot-remove will occur,
       because, even if online_map bit is set, kernel may
       touch NODE_DATA() due to race condition. :-(

495ab9c045 [PATCH] i386/x86-64/ia64: Move polling flag into thread_info_status
During some profiling I noticed that default_idle causes a lot of
memory traffic. I think that is caused by the atomic operations
to clear/set the polling flag in thread_info. There is actually
no reason to make this atomic - only the idle thread does it
to itself, other CPUs only read it. So I moved it into ti->status.

Converted i386/x86-64/ia64 for now because that was the easiest
way to fix ACPI which also manipulates these flags in its idle
function.

e6f47f978b [PATCH] Notify page fault call chain
With this patch Kprobes now registers for page fault notifications only when
their is an active probe registered.  Once all the active probes are
unregistered their is no need to be notified of page faults and kprobes
unregisters itself from the page fault notifications.  Hence we will have ZERO
side effects when no probes are active.

bfe5d83419 [PATCH] Define __raw_get_cpu_var and use it
There are several instances of per_cpu(foo, raw_smp_processor_id()), which
is semantically equivalent to __get_cpu_var(foo) but without the warning
that smp_processor_id() can give if CONFIG_DEBUG_PREEMPT is enabled.  For
those architectures with optimized per-cpu implementations, namely ia64,
powerpc, s390, sparc64 and x86_64, per_cpu() turns into more and slower
code than __get_cpu_var(), so it would be preferable to use __get_cpu_var
on those platforms.

This defines a __raw_get_cpu_var(x) macro which turns into per_cpu(x,
raw_smp_processor_id()) on architectures that use the generic per-cpu
implementation, and turns into __get_cpu_var(x) on the architectures that
have an optimized per-cpu implementation.

742755a1d8 [PATCH] page migration: sys_move_pages(): support moving of individual pages
move_pages() is used to move individual pages of a process. The function can
be used to determine the location of pages and to move them onto the desired
node. move_pages() returns status information for each page.

long move_pages(pid, number_of_pages_to_move,
		addresses_of_pages[],
		nodes[] or NULL,
		status[],
		flags);

The addresses of pages is an array of void * pointing to the
pages to be moved.

The nodes array contains the node numbers that the pages should be moved
to. If a NULL is passed instead of an array then no pages are moved but
the status array is updated. The status request may be used to determine
the page state before issuing another move_pages() to move pages.

The status array will contain the state of all individual page migration
attempts when the function terminates. The status array is only valid if
move_pages() completed successfullly.

Possible page states in status[]:

0..MAX_NUMNODES	The page is now on the indicated node.

-ENOENT		Page is not present

-EACCES		Page is mapped by multiple processes and can only
		be moved if MPOL_MF_MOVE_ALL is specified.

-EPERM		The page has been mlocked by a process/driver and
		cannot be moved.

-EBUSY		Page is busy and cannot be moved. Try again later.

-EFAULT		Invalid address (no VMA or zero page).

-ENOMEM		Unable to allocate memory on target node.

-EIO		Unable to write back page. The page must be written
		back in order to move it since the page is dirty and the
		filesystem does not provide a migration function that
		would allow the moving of dirty pages.

-EINVAL		A dirty page cannot be moved. The filesystem does not provide
		a migration function and has no ability to write back pages.

The flags parameter indicates what types of pages to move:

MPOL_MF_MOVE	Move pages that are only mapped by the process.

MPOL_MF_MOVE_ALL Also move pages that are mapped by multiple processes.
		Requires sufficient capabilities.

Possible return codes from move_pages()

-ENOENT		No pages found that would require moving. All pages
		are either already on the target node, not present, had an
		invalid address or could not be moved because they were
		mapped by multiple processes.

-EINVAL		Flags other than MPOL_MF_MOVE(_ALL) specified or an attempt
		to migrate pages in a kernel thread.

-EPERM		MPOL_MF_MOVE_ALL specified without sufficient priviledges.
		or an attempt to move a process belonging to another user.

-EACCES		One of the target nodes is not allowed by the current cpuset.

-ENODEV		One of the target nodes is not online.

-ESRCH		Process does not exist.

-E2BIG		Too many pages to move.

-ENOMEM		Not enough memory to allocate control array.

-EFAULT		Parameters could not be accessed.

4f1bcaf094 [PATCH] vgacon: make VGA_MAP_MEM take size, remove extra use
VGA_MAP_MEM translates to ioremap() on some architectures.  It makes sense
to do this to vga_vram_base, because we're going to access memory between
vga_vram_base and vga_vram_end.

But it doesn't really make sense to map starting at vga_vram_end, because
we aren't going to access memory starting there.  On ia64, which always has
to be different, ioremapping vga_vram_end gives you something completely
incompatible with ioremapped vga_vram_start, so vga_vram_size ends up being
nonsense.

As a bonus, we often know the size up front, so we can use ioremap()
correctly, rather than giving it a zero size.

d270acbc24 [IA64] Sanitize assembler code for ia64_sal_os_state
struct ia64_sal_os_state has three semi-independent sections.  The code
in mca_asm.S assumes that these three sections are contiguous, which
makes it very awkward to add new data to this structure.  Remove the
assumption that the sections are contiguous.  Define a macro to shorten
references to offsets in ia64_sal_os_state.

This patch does not change the way that the code behaves.  It just
makes it easier to update the code in future and to add fields to
ia64_sal_os_state when debugging the MCA/INIT handlers.

2ab561a116 [IA64] esi-support
Add support for making ESI calls [1].  ESI stands for "Extensible SAL
specification" and is basically a way for invoking firmware
subroutines which are identified by a GUID.  I don't know whether ESI
is used by vendors other than HP (if you do, please let me know) but
as firmware "backdoors" go, this seems one of the cleaner methods, so
it seems reasonable to support it, even though I'm not aware of any
publicly documented ESI calls.  I'd have liked to make the ESI module
completely stand-alone, but unfortunately that is not easily (or not
at all) possible because in order to make ESI calls in physical mode,
a small stub similar to the EFI stub is needed in the kernel proper.
I did try to create a stub that would work in user-level, but it
quickly got ugly beyond recognition (e.g., the stub had to make
assumptions about how the module-loader generated call-stubs work) and
I didn't even get it to work (that's probably fixable, but I didn't
bother because I concluded it was too ugly anyhow).  While it's not
terribly elegant to have kernel code which isn't actively used in the
kernel proper, I think it might be worth making an exception here for
two reasons: the code is trivially small (all that's really needed is
esi_stub.S) and by including it in the normal kernel distro, it might
encourage other OEMs to also use ESI, which I think would be far
better than each inventing their own firmware "backdoor".

The code was originally written by Alex.  I just massaged and packaged
it a bit (and perhaps messed up some things along the way...).

Changes since first version of patch that was posted to mailing list:
* Export ia64_esi_call and ia64_esi_call_phys() as GPL symbols.
* Disallow building esi.c as a module for now.  Building as a module
  would currently lead to an unresolved reference to "sal_lock" on SMP kernels
  because that symbol doesn't get exported.
* Export esi_call_phys() only if ESI is enabled.
* Remove internal stuff from esi.h and add a "proc_type" argument to
  ia64_esi_call() such that serialization-requirements can be expressed (ESI
  follows SAL here, where procedure calls may have to be serialized, are
  MP-safe, or MP-safe andr reentrant).

76d08bb3f0 [IA64] Add "model name" to /proc/cpuinfo
Linux ia64 port tried to decode the processor family number
to something human-readable, but Intel brandnames don't change
synchronously with updates to the family number.  Adopt a more
i386-like approach and just print the family number in decimal.
Add a new field "model name" that uses PAL_BRAND_INFO to find
the official name for the cpu, or on older systems, falls back
to using the well-known codenames (Merced, McKinley, Madison).

32e62c636a [IA64] rework memory attribute aliasing
This closes a couple holes in our attribute aliasing avoidance scheme:

  - The current kernel fails mmaps of some /dev/mem MMIO regions because
    they don't appear in the EFI memory map.  This keeps X from working
    on the Intel Tiger box.

  - The current kernel allows UC mmap of the 0-1MB region of
    /sys/.../legacy_mem even when the chipset doesn't support UC
    access.  This causes an MCA when starting X on HP rx7620 and rx8620
    boxes in the default configuration.

There's more detail in the Documentation/ia64/aliasing.txt file this
adds, but the general idea is that if a region might be covered by
a granule-sized kernel identity mapping, any access via /dev/mem or
mmap must use the same attribute as the identity mapping.

Otherwise, we fall back to using an attribute that is supported
according to the EFI memory map, or to using UC if the EFI memory
map doesn't mention the region.

In SLES10 (2.6.16) crash dumping (in my experience, LKCD) is unable to
capture the second page of the 2-page task/stack allocation.
This is particularly troublesome for dump analysis, as the stack traceback
cannot be done.
  (A similar convention is probably needed throughout the kernel to make
   kernel multi-page allocations detectable for dumping)

Multi-page kernel allocations are represented by the single page structure
associated with the first page of the allocation.  The page structures
associated with the other pages are unintialized.

If the dumper is selecting only kernel pages it has no way to identify
any but the first page of the allocation.

The fix is to make the task/stack allocation a compound page.

912d35f867 [PATCH] Add support for the sys_vmsplice syscall
sys_splice() moves data to/from pipes with a file input/output. sys_vmsplice()
moves data to a pipe, with the input being a user address range instead.

This uses an approach suggested by Linus, where we can hold partial ranges
inside the pages[] map. Hopefully this will be useful for network
receive support as well.

e5ecc192df [IA64] Setup an IA64 specific reclaim distance
RECLAIM_DISTANCE is checked on bootup against the SLIT table distances.
Zone reclaim is important for system that have higher latencies but not for
systems that have multiple nodes on one motherboard and therefore low latencies.

We found that on motherboard latencies are typically 1 to 1.4 of local memory
access speed whereas multinode systems which benefit from zone reclaim have
usually more than 1.5 times the latency of a local access.

Set the reclaim distance for IA64 to 1.5 times.

70524490ee [PATCH] splice: add support for sys_tee()
Basically an in-kernel implementation of tee, which uses splice and the
pipe buffers as an intelligent way to pass data around by reference.

Where the user space tee consumes the input and produces a stdout and
file output, this syscall merely duplicates the data inside a pipe to
another pipe. No data is copied, the output just grabs a reference to the
input pipe data.

c80d79d746 [PATCH] Configurable NODES_SHIFT
Current implementations define NODES_SHIFT in include/asm-xxx/numnodes.h for
each arch.  Its definition is sometimes configurable.  Indeed, ia64 defines 5
NODES_SHIFT values in the current git tree.  But it looks a bit messy.

SGI-SN2(ia64) system requires 1024 nodes, and the number of nodes already has
been changeable by config.  Suitable node's number may be changed in the
future even if it is other architecture.  So, I wrote configurable node's
number.

This patch set defines just default value for each arch which needs multi
nodes except ia64.  But, it is easy to change to configurable if necessary.

On ia64 the number of nodes can be already configured in generic ia64 and SN2
config.  But, NODES_SHIFT is defined for DIG64 and HP'S machine too.  So, I
changed it so that all platforms can be configured via CONFIG_NODES_SHIFT.  It
would be simpler.

The MCA/INIT handlers maintain important state in the SAL to OS (sos)
area and in the monarch_cpu flag.  Kernel debuggers (such as KDB) need
this data, and may need to adjust the monarch_cpu field so make the
data available to the notify_die hooks.  Define two more events for
calling the functions on the notify_die chain.

2db8d99ffd [IA64] always map VGA framebuffer UC, even if it supports WB
EFI on some machines, e.g., Intel Tiger, reports that the VGA framebuffer
supports WB access.  ioremap() prefers WB when possible, so it can work
when mapping main memory.

But it doesn't make sense to map a framebuffer WB, because the driver
doesn't flush explicitly, so updates won't make it to the device
immediately.

This is due to Zou Nan hai <nanhai.zou@intel.com>.

More extensive fix that adds a "size" argument coming soon.

cfab9d0e1d [IA64] fix bug in ia64 __mutex_fastpath_trylock
The parenthesis around "likely" used in ia64 __mutex_fastpath_trylock
is incorrect, and it leads to broken mutex_trylock.  Here is the
patch that fixed the bug.  I removed the likely altogether because
there is no branch and gcc does a reasonable job at predicating the
return value.

5274f052e7 [PATCH] Introduce sys_splice() system call
This adds support for the sys_splice system call. Using a pipe as a
transport, it can connect to files or sockets (latter as output only).

From the splice.c comments:

   "splice": joining two ropes together by interweaving their strands.

   This is the "extended pipe" functionality, where a pipe is used as
   an arbitrary in-memory buffer. Think of a pipe as a small kernel
   buffer that you can use to transfer data from one end to the other.

   The traditional unix read/write is extended with a "splice()" operation
   that transfers data buffers to or from a pipe buffer.

   Named by Larry McVoy, original implementation from Linus, extended by
   Jens to support splicing to files and fixing the initial implementation
   bugs.

e041c68341 [PATCH] Notifier chain update: API changes
The kernel's implementation of notifier chains is unsafe.  There is no
protection against entries being added to or removed from a chain while the
chain is in use.  The issues were discussed in this thread:

We noticed that notifier chains in the kernel fall into two basic usage
classes:

	"Blocking" chains are always called from a process context
	and the callout routines are allowed to sleep;

	"Atomic" chains can be called from an atomic context and
	the callout routines are not allowed to sleep.

We decided to codify this distinction and make it part of the API.  Therefore
this set of patches introduces three new, parallel APIs: one for blocking
notifiers, one for atomic notifiers, and one for "raw" notifiers (which is
really just the old API under a new name).  New kinds of data structures are
used for the heads of the chains, and new routines are defined for
registration, unregistration, and calling a chain.  The three APIs are
explained in include/linux/notifier.h and their implementation is in
kernel/sys.c.

With atomic and blocking chains, the implementation guarantees that the chain
links will not be corrupted and that chain callers will not get messed up by
entries being added or removed.  For raw chains the implementation provides no
guarantees at all; users of this API must provide their own protections.  (The
idea was that situations may come up where the assumptions of the atomic and
blocking APIs are not appropriate, so it should be possible for users to
handle these things in their own way.)

There are some limitations, which should not be too hard to live with.  For
atomic/blocking chains, registration and unregistration must always be done in
a process context since the chain is protected by a mutex/rwsem.  Also, a
callout routine for a non-raw chain must not try to register or unregister
entries on its own chain.  (This did happen in a couple of places and the code
had to be changed to avoid it.)

Since atomic chains may be called from within an NMI handler, they cannot use
spinlocks for synchronization.  Instead we use RCU.  The overhead falls almost
entirely in the unregister routine, which is okay since unregistration is much
less frequent that calling a chain.

of them use the raw API, so for the moment it is only a placeholder.

gets called very frequently should be atomic, because the rwsem read-locking

atomic.)

b2c99e3c70 [PATCH] EFI: keep physical table addresses in efi structure
Almost all users of the table addresses from the EFI system table want
physical addresses.  So rather than doing the pa->va->pa conversion, just keep
physical addresses in struct efi.

This fixes a DMI bug: the efi structure contained the physical SMBIOS address
on x86 but the virtual address on ia64, so dmi_scan_machine() used ioremap()
on a virtual address on ia64.

This is essentially the same as an earlier patch by Matt Tolentino:

except that this changes all table addresses, not just ACPI addresses.

Matt's original patch was backed out because it caused MCAs on HP sx1000
systems.  That problem is resolved by the ioremap() attribute checking added

Pass the size, not a pointer to the size, to efi_mem_attribute_range().

This function validates memory regions for the /dev/mem read/write/mmap paths.
The pointer allows arches to reduce the size of the range, but I think that's
unnecessary complexity.  Simplifying it will let me use
efi_mem_attribute_range() to improve the ia64 ioremap() implementation.

3ed3bce846 [PATCH] ia64: use i386 dmi_scan.c
Enable DMI table parsing on ia64.

Andi Kleen has a patch in his x86_64 tree which enables the use of i386
dmi_scan.c on x86_64.  dmi_scan.c functions are being used by the
drivers/char/ipmi/ipmi_si_intf.c driver for autodetecting the ports or
memory spaces where the IPMI controllers may be found.

This patch adds equivalent changes for ia64 as to what is in the x86_64
tree.  In addition, I reworked the DMI detection, such that on EFI-capable
systems, it uses the efi.smbios pointer to find the table, rather than
brute-force searching from 0xF0000.  On non-EFI systems, it continues the
brute-force search.

My test system, an Intel S870BN4 'Tiger4', aka Dell PowerEdge 7250, with
latest BIOS, does not list the IPMI controller in the ACPI namespace, nor
does it have an ACPI SPMI table.  Also note, currently shipping Dell x8xx
EM64T servers don't have these either, so DMI is the only method for
obtaining the address of the IPMI controller.

f348d70a32 [PATCH] POLLRDHUP/EPOLLRDHUP handling for half-closed devices notifications
Implement the half-closed devices notifiation, by adding a new POLLRDHUP
(and its alias EPOLLRDHUP) bit to the existing poll/select sets.  Since the
existing POLLHUP handling, that does not report correctly half-closed
devices, was feared to be changed, this implementation leaves the current
POLLHUP reporting unchanged and simply add a new bit that is set in the few
places where it makes sense.  The same thing was discussed and conceptually
agreed quite some time ago:

Since this new event bit is added to the existing Linux poll infrastruture,
even the existing poll/select system calls will be able to use it.  As far
as the existing POLLHUP handling, the patch leaves it as is.  The
pollrdhup-2.6.16.rc5-0.10.diff defines the POLLRDHUP for all the existing
archs and sets the bit in the six relevant files.  The other attached diff
is the simple change required to sys/epoll.h to add the EPOLLRDHUP
definition.

There is "a stupid program" to test POLLRDHUP delivery here:

It tests poll(2), but since the delivery is same epoll(2) will work equally.

4129a953ad [IA64] New IA64 core/thread detection patch
IPF SDM 2.2 changes definition of PAL_LOGICAL_TO_PHYSICAL to add
proc_number=-1 to get core/thread mapping info on the running processer.

Based on this change, we had better to update existing core/thread
detection in IA64 kernel correspondingly. The attached patch implements
this change. It simplifies detection code and eliminates potential race
condition. It also runs a bit faster and has better scalability especially
when cores and threads number grows up in one package.

3ad5ef8b9d [IA64] Increase max node count on SN platforms
Add support in IA64 acpi for platforms that support more than
256 nodes. Currently, ACPI is limited to 256 nodes because the
proximity domain number is 8-bits.

Long term, we expect to use ACPI3.0 to support >256 nodes.
This patch is an interim solution that works with platforms
that pass the  high order bits of the proximity domain in
"reserved" fields of the ACPI tables. This code is enabled
ONLY on SN platforms.

d2a28ad9fa [IA64] MCA recovery: kernel context recovery table
Memory errors encountered by user applications may surface
when the CPU is running in kernel context.  The current code
will not attempt recovery if the MCA surfaces in kernel
context (privilage mode 0).  This patch adds a check for cases
where the user initiated the load that surfaces in kernel
interrupt code.

An example is a user process lauching a load from memory
and the data in memory had bad ECC.  Before the bad data
gets to the CPU register, and interrupt comes in.  The
code jumps to the IVT interrupt entry point and begins
execution in kernel context.  The process of saving the
user registers (SAVE_REST) causes the bad data to be loaded
into a CPU register, triggering the MCA.  The MCA surfaces in
kernel context, even though the load was initiated from
user context.

As suggested by David and Tony, this patch uses an exception
table like approach, puting the tagged recovery addresses in
a searchable table.  One difference from the exception table
is that MCAs do not surface in precise places (such as with
a TLB miss), so instead of tagging specific instructions,
address ranges are registers.  A single macro is used to do
the tagging, with the input parameter being the label
of the starting address and the macro being the ending
address.  This limits clutter in the code.

This patch only tags one spot, the interrupt ivt entry.
Testing showed that spot to be a "heavy hitter" with
MCAs surfacing while saving user registers.  Other spots
can be added as needed by adding a single macro.

804f1594cc [PATCH] Move read_mostly definition to asm/cache.h
Seems like needless clutter having a bunch of #if defined(CONFIG_$ARCH) in
include/linux/cache.h.  Move the per architecture section definition to
asm/cache.h, and keep the if-not-defined dummy case in linux/cache.h to
catch architectures which don't implement the section.

Verified that symbols still go in .data.read_mostly on parisc,
and the compile doesn't break.

42b88befd6 [PATCH] hugepage: is_aligned_hugepage_range() cleanup
Quite a long time back, prepare_hugepage_range() replaced
is_aligned_hugepage_range() as the callback from mm/mmap.c to arch code to
verify if an address range is suitable for a hugepage mapping.
is_aligned_hugepage_range() stuck around, but only to implement
prepare_hugepage_range() on archs which didn't implement their own.

Most archs (everything except ia64 and powerpc) used the same
implementation of is_aligned_hugepage_range().  On powerpc, which
implements its own prepare_hugepage_range(), the custom version was never
used.

In addition, "is_aligned_hugepage_range()" was a bad name, because it
suggests it returns true iff the given range is a good hugepage range,
whereas in fact it returns 0-or-error (so the sense is reversed).

This patch cleans up by abolishing is_aligned_hugepage_range().  Instead
prepare_hugepage_range() is defined directly.  Most archs use the default
version, which simply checks the given region is aligned to the size of a
hugepage.  ia64 and powerpc define custom versions.  The ia64 one simply
checks that the range is in the correct address space region in addition to
being suitably aligned.  The powerpc version (just as previously) checks
for suitable addresses, and if necessary performs low-level MMU frobbing to
set up new areas for use by hugepages.

No libhugetlbfs testsuite regressions on ppc64 (POWER5 LPAR).

The optional hugepage callback, hugetlb_free_pgd_range() is presently
implemented non-trivially only on ia64 (but I plan to add one for powerpc
shortly).  It has its own prototype for the function in asm-ia64/pgtable.h.
 However, since the function is called from generic code, it make sense for
its prototype to be in the generic hugetlb.h header file, as the protypes
other arch callbacks already are (prepare_hugepage_range(),
set_huge_pte_at(), etc.).  This patch makes it so.

9da61aef0f [PATCH] hugepage: Fix hugepage logic in free_pgtables()
free_pgtables() has special logic to call hugetlb_free_pgd_range() instead
of the normal free_pgd_range() on hugepage VMAs.  However, the test it uses
to do so is incorrect: it calls is_hugepage_only_range on a hugepage sized
range at the start of the vma.  is_hugepage_only_range() will return true
if the given range has any intersection with a hugepage address region, and
in this case the given region need not be hugepage aligned.  So, for
example, this test can return true if called on, say, a 4k VMA immediately
preceding a (nicely aligned) hugepage VMA.

At present we get away with this because the powerpc version of
hugetlb_free_pgd_range() is just a call to free_pgd_range().  On ia64 (the
only other arch with a non-trivial is_hugepage_only_range()) we get away
with it for a different reason; the hugepage area is not contiguous with
the rest of the user address space, and VMAs are not permitted in between,
so the test can't return a false positive there.

Nonetheless this should be fixed.  We do that in the patch below by
replacing the is_hugepage_only_range() test with an explicit test of the
VMA using is_vm_hugetlb_page().

This in turn changes behaviour for platforms where is_hugepage_only_range()
returns false always (everything except powerpc and ia64).  We address this
by ensuring that hugetlb_free_pgd_range() is defined to be identical to
free_pgd_range() (instead of a no-op) on everything except ia64.  Even so,
it will prevent some otherwise possible coalescing of calls down to
free_pgd_range().  Since this only happens for hugepage VMAs, removing this
small optimization seems unlikely to cause any trouble.

This patch causes no regressions on the libhugetlbfs testsuite - ppc64
POWER5 (8-way), ppc64 G5 (2-way) and i386 Pentium M (UP).

2.6.16-rc3 uses hugetlb on-demand paging, but it doesn_t support hugetlb
mprotect.

  Remove a test from the mprotect() path which checks that the mprotect()ed
  range on a hugepage VMA is hugepage aligned (yes, really, the sense of
  is_aligned_hugepage_range() is the opposite of what you'd guess :-/).

  In fact, we don't need this test.  If the given addresses match the
  beginning/end of a hugepage VMA they must already be suitably aligned.  If
  they don't, then mprotect_fixup() will attempt to split the VMA.  The very
  first test in split_vma() will check for a badly aligned address on a
  hugepage VMA and return -EINVAL if necessary.

  On i386 and x86-64, pte flag _PAGE_PSE collides with _PAGE_PROTNONE.  The
  identify of hugetlb pte is lost when changing page protection via mprotect.
  A page fault occurs later will trigger a bug check in huge_pte_alloc().

  The fix is to always make new pte a hugetlb pte and also to clean up
  legacy code where _PAGE_PRESENT is forced on in the pre-faulting day.

defbb2c929 [IA64] ia64: simplify and fix udelay()
The original ia64 udelay() was simple, but flawed for platforms without
synchronized ITCs:  a preemption and migration to another CPU during the
while-loop likely resulted in too-early termination or very, very
lengthy looping.

The first fix (now in 2.6.15) broke the delay loop into smaller,
non-preemptible chunks, reenabling preemption between the chunks.  This
fix is flawed in that the total udelay is computed to be the sum of just
the non-premptible while-loop pieces, i.e., not counting the time spent
in the interim preemptible periods.  If an interrupt or a migration
occurs during one of these interim periods, then that time is invisible
and only serves to lengthen the effective udelay().

This new fix backs out the current flawed fix and returns to a simple
udelay(), fully preemptible and interruptible.  It implements two simple
alternative udelay() routines:  one a default generic version that uses
ia64_get_itc(), and the other an sn-specific version that uses that
platform's RTC.

f822566165 [PATCH] madvise MADV_DONTFORK/MADV_DOFORK
Currently, copy-on-write may change the physical address of a page even if the
user requested that the page is pinned in memory (either by mlock or by
get_user_pages).  This happens if the process forks meanwhile, and the parent
writes to that page.  As a result, the page is orphaned: in case of
get_user_pages, the application will never see any data hardware DMA's into
this page after the COW.  In case of mlock'd memory, the parent is not getting
the realtime/security benefits of mlock.

In particular, this affects the Infiniband modules which do DMA from and into
user pages all the time.

This patch adds madvise options to control whether memory range is inherited
across fork.  Useful e.g.  for when hardware is doing DMA from/into these
pages.  Could also be useful to an application wanting to speed up its forks
by cutting large areas out of consideration.

Have a facility to account for potentially hot-pluggable CPUs. ACPI doesnt
give a determinstic method to find hot-pluggable CPUs. Hence we use 2 methods

- BIOS can mark potentially hot-pluggable CPUs as disabled in the MADT tables.
- User can specify the number of hot-pluggable CPUs via parameter
  additional_cpus=X

The option is enabled only if ACPI_CONFIG_HOTPLUG_CPU=y which enables the
physical hotplug option. Without which user can still use logical onlining
and offlining of CPUs by enabling CONFIG_HOTPLUG_CPU=y

Adds more bits to cpu_possible_map for potentially hot-pluggable cpus.

dcc1dd2366 [IA64-SGI] - Eliminate SN pio_phys_xxx macros. Move to assembly
Rewrite the SN pio_phys_xxx macros in assembly language. This
avoids issues with the Intel icc compiler. Function call
overhead is not an issue - the functions reference PIOs
and take 100's nsec to complete.

In addition, the functions should likely be in assembly
language anyway - they reference memory using physical
addressing mode. One function executes with psr.ic disabled.

69dcc99199 [PATCH] Export cpu topology in sysfs
The patch implements cpu topology exportation by sysfs.

Items (attributes) are similar to /proc/cpuinfo.

1) /sys/devices/system/cpu/cpuX/topology/physical_package_id:
	represent the physical package id of  cpu X;
2) /sys/devices/system/cpu/cpuX/topology/core_id:
	represent the cpu core id to cpu X;
3) /sys/devices/system/cpu/cpuX/topology/thread_siblings:
	represent the thread siblings to cpu X in the same core;
4) /sys/devices/system/cpu/cpuX/topology/core_siblings:
	represent the thread siblings to cpu X in the same physical package;

To implement it in an architecture-neutral way, a new source file,
driver/base/topology.c, is to export the 5 attributes.

If one architecture wants to support this feature, it just needs to
implement 4 defines, typically in file include/asm-XXX/topology.h.
The 4 defines are:
#define topology_physical_package_id(cpu)
#define topology_core_id(cpu)
#define topology_thread_siblings(cpu)
#define topology_core_siblings(cpu)

The type of **_id is int.
The type of siblings is cpumask_t.

To be consistent on all architectures, the 4 attributes should have
deafult values if their values are unavailable. Below is the rule.

1) physical_package_id: If cpu has no physical package id, -1 is the
default value.

2) core_id: If cpu doesn't support multi-core, its core id is 0.

3) thread_siblings: Just include itself, if the cpu doesn't support
HT/multi-thread.

4) core_siblings: Just include itself, if the cpu doesn't support
multi-core and HT/Multi-thread.

So be careful when declaring the 4 defines in include/asm-XXX/topology.h.

If an attribute isn't defined on an architecture, it won't be exported.

a58786917c [IA64] avoid broken SAL_CACHE_FLUSH implementations
If SAL_CACHE_FLUSH drops interrupts, complain about it and fall back to
using PAL_CACHE_FLUSH instead.

This is to work around a defect in HP rx5670 firmware: when an interrupt
occurs during SAL_CACHE_FLUSH, SAL drops the interrupt but leaves it marked
"in-service", which leaves the interrupt (and others of equal or lower
priority) masked.

e08e6c5213 [IA64] hooks to wait for mmio writes to drain when migrating processes
On SN2, MMIO writes which are issued from separate processors are not
guaranteed to arrive in any particular order at the IO hardware.  When
performing such writes from the kernel this is not a problem, as a
kernel thread will not migrate to another CPU during execution, and
mmiowb() calls can guarantee write ordering when control of the IO
resource is allowed to move between threads.

However, when MMIO writes can be performed from user space (e.g. DRM)
there are no such guarantees and mechanisms, as the process may
context-switch at any time, and may migrate to a different CPU as part
of the switch.  For such programs/hardware to operate correctly, it is
required that the MMIO writes from the old CPU be accepted by the IO
hardware before subsequent writes from the new CPU can be issued.

The following patch implements this behavior on SN2 by waiting for a
Shub register to indicate that these writes have been accepted.  This
is placed in the context switch-in path, and only performs the wait
when the newly scheduled task changes CPUs.

Add driver support for a 2 port PCI IOC3-based serial card on Altix boxes:

6d6e420005 [IA64-SGI] Fix sn_flush_device_kernel & spinlock initialization
This patch separates the sn_flush_device_list struct into kernel and
common (both kernel and PROM accessible) structures.  As it was, if the
size of a spinlock_t changed (due to additional CONFIG options, etc.) the
sal call which populated the sn_flush_device_list structs would erroneously
write data (and cause memory corruption and/or a panic).

This patch does the following:

1.  Removes sn_flush_device_list and adds sn_flush_device_common and
sn_flush_device_kernel.

2.  Adds a new SAL call to populate a sn_flush_device_common struct per
device, not per widget as previously done.

3.  Correctly initializes each device's sn_flush_device_kernel spinlock_t
struct (before it was only doing each widget's first device).

This patch fixes a problem in XPC disengage processing whereby it was not
seeing the request to disengage from a remote partition, so the disengage
wasn't happening. The disengagement is suppose to transpire during the time

This is the latest version of the scheduler cache-hot-auto-tune patch.

The first problem was that detection time scaled with O(N^2), which is
unacceptable on larger SMP and NUMA systems. To solve this:

- I've added a 'domain distance' function, which is used to cache
  measurement results. Each distance is only measured once. This means
  that e.g. on NUMA distances of 0, 1 and 2 might be measured, on HT
  distances 0 and 1, and on SMP distance 0 is measured. The code walks
  the domain tree to determine the distance, so it automatically follows
  whatever hierarchy an architecture sets up. This cuts down on the boot
  time significantly and removes the O(N^2) limit. The only assumption
  is that migration costs can be expressed as a function of domain
  distance - this covers the overwhelming majority of existing systems,
  and is a good guess even for more assymetric systems.

  [ People hacking systems that have assymetries that break this
    assumption (e.g. different CPU speeds) should experiment a bit with
    the cpu_distance() function. Adding a ->migration_distance factor to
    the domain structure would be one possible solution - but lets first
    see the problem systems, if they exist at all. Lets not overdesign. ]

Another problem was that only a single cache-size was used for measuring
the cost of migration, and most architectures didnt set that variable
up. Furthermore, a single cache-size does not fit NUMA hierarchies with
L3 caches and does not fit HT setups, where different CPUs will often
have different 'effective cache sizes'. To solve this problem:

- Instead of relying on a single cache-size provided by the platform and
  sticking to it, the code now auto-detects the 'effective migration
  cost' between two measured CPUs, via iterating through a wide range of
  cachesizes. The code searches for the maximum migration cost, which
  occurs when the working set of the test-workload falls just below the
  'effective cache size'. I.e. real-life optimized search is done for
  the maximum migration cost, between two real CPUs.

  This, amongst other things, has the positive effect hat if e.g. two
  CPUs share a L2/L3 cache, a different (and accurate) migration cost
  will be found than between two CPUs on the same system that dont share
  any caches.

(The reliable measurement of migration costs is tricky - see the source
for details.)

Furthermore i've added various boot-time options to override/tune
migration behavior.

Firstly, there's a blanket override for autodetection:

	migration_cost=1000,2000,3000

will override the depth 0/1/2 values with 1msec/2msec/3msec values.

Secondly, there's a global factor that can be used to increase (or
decrease) the autodetected values:

	migration_factor=120

will increase the autodetected values by 20%. This option is useful to
tune things in a workload-dependent way - e.g. if a workload is
cache-insensitive then CPU utilization can be maximized by specifying
migration_factor=0.

I've tested the autodetection code quite extensively on x86, on 3
P3/Xeon/2MB, and the autodetected values look pretty good:

49a2a1b83b [PATCH] kprobes: changed from using spinlock to mutex
Since Kprobes runtime exception handlers is now lock free as this code path is
now using RCU to walk through the list, there is no need for the
register/unregister{_kprobe} to use spin_{lock/unlock}_isr{save/restore}.  The
serialization during registration/unregistration is now possible using just a
mutex.

In the above process, this patch also fixes a minor memory leak for x86_64 and
powerpc.

2acbb8c657 [PATCH] mutex subsystem, add default include/asm-*/mutex.h files
add the per-arch mutex.h files for the remaining architectures.

We default to asm-generic/mutex-dec.h, because that performs
quite well on most arches. Arches that do not have atomic
decrement/increment instructions should switch to mutex-xchg.h
instead. Arches can also provide their own implementation for
the mutex fastpath primitives.

80851ef2a5 [PATCH] /dev/mem: validate mmap requests
Add a hook so architectures can validate /dev/mem mmap requests.

This is analogous to validation we already perform in the read/write
paths.

The identity mapping scheme used on ia64 requires that each 16MB or
64MB granule be accessed with exactly one attribute (write-back or
uncacheable).  This avoids "attribute aliasing", which can cause a
machine check.

Sample problem scenario:
  - Machine supports VGA, so it has uncacheable (UC) MMIO at 640K-768K
  - efi_memmap_init() discards any write-back (WB) memory in the first granule
  - Application (e.g., "hwinfo") mmaps /dev/mem, offset 0
  - hwinfo receives UC mapping (the default, since memmap says "no WB here")
  - Machine check abort (on chipsets that don't support UC access to WB
    memory, e.g., sx1000)

In the scenario above, the only choices are
  - Use WB for hwinfo mmap.  Can't do this because it causes attribute
    aliasing with the UC mapping for the VGA MMIO space.
  - Use UC for hwinfo mmap.  Can't do this because the chipset may not
    support UC for that region.
  - Disallow the hwinfo mmap with -EINVAL.  That's what this patch does.

39743889aa [PATCH] Swap Migration V5: sys_migrate_pages interface
sys_migrate_pages implementation using swap based page migration

This is the original API proposed by Ray Bryant in his posts during the first
half of 2005 on linux-mm@kvack.org and linux-kernel@vger.kernel.org.

The intent of sys_migrate is to migrate memory of a process.  A process may
have migrated to another node.  Memory was allocated optimally for the prior
context.  sys_migrate_pages allows to shift the memory to the new node.

sys_migrate_pages is also useful if the processes available memory nodes have
changed through cpuset operations to manually move the processes memory.  Paul
Jackson is working on an automated mechanism that will allow an automatic
migration if the cpuset of a process is changed.  However, a user may decide
to manually control the migration.

This implementation is put into the policy layer since it uses concepts and
functions that are also needed for mbind and friends.  The patch also provides
a do_migrate_pages function that may be useful for cpusets to automatically
move memory.  sys_migrate_pages does not modify policies in contrast to Ray's
implementation.

The current code here is based on the swap based page migration capability and
thus is not able to preserve the physical layout relative to it containing
nodeset (which may be a cpuset).  When direct page migration becomes available
then the implementation needs to be changed to do a isomorphic move of pages
between different nodesets.  The current implementation simply evicts all
pages in source nodeset that are not in the target nodeset.

Patch supports ia64, i386 and x86_64.

d3cb487149 [PATCH] atomic_long_t & include/asm-generic/atomic.h V2
Several counters already have the need to use 64 atomic variables on 64 bit
platforms (see mm_counter_t in sched.h).  We have to do ugly ifdefs to fall
back to 32 bit atomic on 32 bit platforms.

The VM statistics patch that I am working on will also make more extensive
use of atomic64.

This patch introduces a new type atomic_long_t by providing definitions in
asm-generic/atomic.h that works similar to the c "long" type.  Its 32 bits
on 32 bit platforms and 64 bits on 64 bit platforms.

Also cleans up the determination of the mm_counter_t in sched.h.

f6b3ec238d [PATCH] madvise(MADV_REMOVE): remove pages from tmpfs shm backing store
Here is the patch to implement madvise(MADV_REMOVE) - which frees up a
given range of pages & its associated backing store.  Current
implementation supports only shmfs/tmpfs and other filesystems return
-ENOSYS.

"Some app allocates large tmpfs files, then when some task quits and some
client disconnect, some memory can be released.  However the only way to
release tmpfs-swap is to MADV_REMOVE". - Andrea Arcangeli

Databases want to use this feature to drop a section of their bufferpool
(shared memory segments) - without writing back to disk/swap space.

This feature is also useful for supporting hot-plug memory on UML.

Concerns raised by Andrew Morton:

- "We have no plan for holepunching!  If we _do_ have such a plan (or
  might in the future) then what would the API look like?  I think
  sys_holepunch(fd, start, len), so we should start out with that."

- Using madvise is very weird, because people will ask "why do I need to
  mmap my file before I can stick a hole in it?"

- None of the other madvise operations call into the filesystem in this
  manner.  A broad question is: is this capability an MM operation or a
  filesytem operation?  truncate, for example, is a filesystem operation
  which sometimes has MM side-effects.  madvise is an mm operation and with
  this patch, it gains FS side-effects, only they're really, really
  significant ones."

Comments:

- Andrea suggested the fs operation too but then it's more efficient to
  have it as a mm operation with fs side effects, because they don't
  immediatly know fd and physical offset of the range.  It's possible to
  fixup in userland and to use the fs operation but it's more expensive,
  the vmas are already in the kernel and we can use them.

Short term plan &  Future Direction:

- We seem to need this interface only for shmfs/tmpfs files in the short
  term.  We have to add hooks into the filesystem for correctness and
  completeness.  This is what this patch does.

- In the future, plan is to support both fs and mmap apis also.  This
  also involves (other) filesystem specific functions to be implemented.

- Current patch doesn't support VM_NONLINEAR - which can be addressed in
  the future.

f5899b5d4f [IA64] disable preemption in udelay()
The udelay() inline for ia64 uses the ITC.  If CONFIG_PREEMPT is enabled
and the platform has unsynchronized ITCs and the calling task migrates
to another CPU while doing the udelay loop, then the effective delay may
be too short or very, very long.

This patch disables preemption around 100 usec chunks of the overall
desired udelay time.  This minimizes preemption-holdoffs.

udelay() is now too big to be inline, move it out of line and export it.

eed66cfcbb [IA64] Split 16-bit severity field in sal_log_record_header
ERR_SEVERITY item is defined as a 8 bits item in SAL documentation
($B.2.1 rev december 2003), but as an u16 in sal.h.
This has the side effect that current code in mca.c may not call
ia64_sal_clear_state_info() upon receiving corrected platform errors
if there are bits set in the validation byte.  Reported by Xavier Bru.

bd1d6e2451 [IA64] Change SET_PERSONALITY to comply with comment in binfmt_elf.c.
We have a customer application which trips a bug.  The problem arises
when a driver attempts to call do_munmap on an area which is mapped, but
because current->thread.task_size has been set to 0xC0000000, the call
to do_munmap fails thinking it is an unmap beyond the user's address
space.

The comment in fs/binfmt_elf.c in load_elf_library() before the call
to SET_PERSONALITY() indicates that task_size must not be changed for
the running application until flush_thread, but is for ia64 executing
ia32 binaries.

This patch moves the setting of task_size from SET_PERSONALITY() to
flush_thread() as indicated.  The customer application no longer is able
to trip the bug.

A single SGI Altix system can be divided into multiple partitions,
each running their own instance of the Linux kernel.  pfn_valid()
is currently not optimal for any but the first partition, since it

some benchmarks and found nothing interesting.  Performance has
consistently fallen within the noise range.

It also introduces a config option (setting the default to 3
levels).  The config option prevents having 4 level page
tables with 64k base page size.

b4033c1715 [PATCH] PCI: Change MSI to use physical delivery mode always
MSI hardcoded delivery mode to use logical delivery mode. Recently
x86_64 moved to use physical mode addressing to support physflat mode.
With this mode enabled noticed that my eth with MSI werent working.

msi_address_init()  was hardcoded to use logical mode for i386 and x86_64.
So when we switch to use physical mode, things stopped working.

Since anyway we dont use lowest priority delivery with MSI, its always
directed to just a single CPU. Its safe  and simpler to use
physical mode always, even when we use logical delivery mode for IPI's
or other ioapic RTE's.

9138d581b0 [IA64] Extend notify_die() hooks for IA64
notify_die() added for MCA_{MONARCH,SLAVE,RENDEZVOUS}_{ENTER,PROCESS,LEAVE} and
INIT_{MONARCH,SLAVE}_{ENTER,PROCESS,LEAVE}.  We need multiple
notification points for these events because they can take many seconds
to run which has nasty effects on the behaviour of the rest of the
system.

DIE_SS replaced by a generic DIE_FAULT which checks the vector number,
to allow interception of faults other than SS.

DIE_MACHINE_{HALT,RESTART} added to allow last minute close down
processing, especially when the halt/restart routines are called from
error handlers.

DIE_OOPS added.

The check for kprobe's break numbers has been moved from traps.c to
kprobes.c, allowing DIE_BREAK to be used for any additional break
numbers, i.e. it is no longer kprobes specific.

Hooks for kernel debuggers and kernel dumpers added, ENTER and LEAVE.
Both of these disable the system for long periods which impact on
watchdogs and heartbeat systems in general.  More patches to come that
use these events to reset watchdogs and heartbeats.

unregister_die_notifier() added and both routines exported.  Requested
by Dean Nelson.

Lock removed from {un,}register_die_notifier.  notifier_chain_register()

481bed4542 [PATCH] consolidate sys_ptrace()
The sys_ptrace boilerplate code (everything outside the big switch
statement for the arch-specific requests) is shared by most architectures.
This patch moves it to kernel/ptrace.c and leaves the arch-specific code as
arch_ptrace.

Some architectures have a too different ptrace so we have to exclude them.
They continue to keep their implementations.  For sh64 I had to add a
sh64_ptrace wrapper because it does some initialization on the first call.
For um I removed an ifdefed SUBARCH_PTRACE_SPECIAL block, but
SUBARCH_PTRACE_SPECIAL isn't defined anywhere in the tree.

e1531b4218 [PATCH] ia64: re-implement dma_get_cache_alignment to avoid EXPORT_SYMBOL
The current ia64 implementation of dma_get_cache_alignment does not work
for modules because it relies on a symbol which is not exported.  Direct
access to a global is a little ugly anyway, so this patch re-implements
dma_get_cache_alignment in a manner similar to what is currently used for
x86_64.

58cd908299 [IA64] make mmu_context.h and tlb.c 80-column friendly
wrap_mmu_context(), delayed_tlb_flush(), get_mmu_context() all
have an extra { } block which cause one extra indentation.
get_mmu_context() is particularly bad with 5 indentations to
the most inner "if".  It finally gets on my nerve that I can't
keep the code within 80 columns.  Remove the extra { } block
and while I'm at it, reformat all the comments to 80-column
friendly.  No functional change at all with this patch.

dcc17d1bae [IA64] Use bitmaps for efficient context allocation/free
Corrects the very inefficent method of finding free context_ids in
get_mmu_context().  Instead of walking the task_list of all processes,
2 bitmaps are used to efficently store and lookup state, inuse and
needs flushing. The entire rid address space is now used before calling
wrap_mmu_context and global tlb flushing.

Special thanks to Ken and Rohit for their review and modifications in
using a bit flushmap.

fc2acab31b [PATCH] mm: tlb_finish_mmu forget rss
zap_pte_range has been counting the pages it frees in tlb->freed, then
tlb_finish_mmu has used that to update the mm's rss.  That got stranger when I
added anon_rss, yet updated it by a different route; and stranger when rss and
anon_rss became mm_counters with special access macros.  And it would no
longer be viable if we're relying on page_table_lock to stabilize the
mm_counter, but calling tlb_finish_mmu outside that lock.

Remove the mmu_gather's freed field, let tlb_finish_mmu stick to its own
business, just decrement the rss mm_counter in zap_pte_range (yes, there was
some point to batching the update, and a subsequent patch restores that).  And
forget the anal paranoia of first reading the counter to avoid going negative
- if rss does go negative, just fix that bug.

Remove the mmu_gather's flushes and avoided_flushes from arm and arm26: no use
was being made of them.  But arm26 alone was actually using the freed, in the
way some others use need_flush: give it a need_flush.  arm26 seems to prefer
spaces to tabs here: respect that.

4d6ddfa924 [PATCH] mm: tlb_is_full_mm was obscure
tlb_is_full_mm?  What does that mean?  The TLB is full?  No, it means that the
mm's last user has gone and the whole mm is being torn down.  And it's an
inline function because sparc64 uses a different (slightly better)
"tlb_frozen" name for the flag others call "fullmm".

And now the ptep_get_and_clear_full macro used in zap_pte_range refers
directly to tlb->fullmm, which would be wrong for sparc64.  Rather than
correct that, I'd prefer to scrap tlb_is_full_mm altogether, and change
sparc64 to just use the same poor name as everyone else - is that okay?

15a23ffa2f [PATCH] mm: tlb_gather_mmu get_cpu_var
tlb_gather_mmu dates from before kernel preemption was allowed, and uses
smp_processor_id or __get_cpu_var to find its per-cpu mmu_gather.  That works
because it's currently only called after getting page_table_lock, which is not
dropped until after the matching tlb_finish_mmu.  But don't rely on that, it
will soon change: now disable preemption internally by proper get_cpu_var in
tlb_gather_mmu, put_cpu_var in tlb_finish_mmu.

c1902aae32 [IA64] - Avoid slow TLB purges on SGI Altix systems
flush_tlb_all() can be a scaling issue on large SGI Altix systems
since it uses the global call_lock and always executes on all cpus.
When a process enters flush_tlb_range() to purge TLBs for another
process, it is possible to avoid flush_tlb_all() and instead allow
sn2_global_tlb_purge() to purge TLBs only where necessary.

This patch modifies flush_tlb_range() so that this case can be handled
by platform TLB purge functions and updates ia64_global_tlb_purge()
accordingly.  sn2_global_tlb_purge() now calculates the region register
value from the mm argument introduced with this patch.

61b9cf7c6c [IA64-SGI] fix sn_pci_legacy_read/fix sn_pci_legacy_write
This patch adds a #define for SN_SAL_IOIF_PCI_SAFE and makes that the
preferred method of implementing sn_pci_legacy_read() and
sn_pci_legacy_write().

This SAL call has been present in SGI proms since version 4.10.  If the
SN_SAL_IOIF_PCI_SAFE call fails, revert to the previous code for compatability
with older proms.

IA64_KR(CURRENT_STACK).  This gives the MCA/INIT handler stacks the

data on return from OS to SAL.

676067cfea [PATCH] Remove unused var from asm/futex.h
As recently done by Russell King for ARM, commit
4732efbeb9 introduces a generic asm/futex.h copied
along most arches, which includes a "-ENOSYS support" to be changed if needed.
However, it includes an unused var (taken from the "real" version) which GCC
warns about.

Remove it from all arches having that file version (i.e. same GIT id).
$ git-diff-tree -r HEAD
and
$ git-ls-tree  -r HEAD include/|grep 9feff4ce14

may be more interesting than looking at the patch itself, to make sure I've
just copied the arm header to all other archs having the original dummy version
of this file.

24ee0a6d7b [IA64] Cleanup use of various #defines related to nodes
Some of the SN code & #defines related to compact nodes & IO discovery
have gotten stale over the years. This patch attempts to clean them up.
Some of the various SN MAX_xxx #defines were also unclear & misused.

The primary changes are:

	- use MAX_NUMNODES. This is the generic linux #define for the number
	  of nodes that are known to the generic kernel. Arrays & loops
	  for constructs that are 1:1 with linux-defined nodes should
	  use the linux #define - not an SN equivalent.

	- use MAX_COMPACT_NODES for MAX_NUMNODES + NUM_TIOS. This is the
	  number of nodes in the SSI system. Compact nodes are a hack to
	  get around the IA64 architectural limit of 256 nodes. Large SGI
	  systems have more than 256 nodes. When we upgrade to ACPI3.0,
	  I _hope_ that all nodes will be real nodes that are known to
	  the generic kernel. That will allow us to delete the notion
	  of "compact nodes".

	- add MAX_NUMALINK_NODES for the total number of nodes that
	  are in the numalink domain - all partitions.

	- simplified (understandable) scan_for_ionodes()

	- small amount of cleanup related to cnodes

0b9afede3d [IA64] more robust zx1/sx1000 machvec support
Machine vector selection has always been a bit of a hack given how
early in system boot it needs to be done.  Services like ACPI namespace
are not available and there are non-trivial problems to moving them to
early boot.  However, there's no reason we can't change to a different
machvec later in boot when the services we need are available.  By
adding a entry point for later initialization of the swiotlb, we can add
an error path for the hpzx1 machevec initialization and fall back to the
DIG machine vector if IOMMU hardware isn't found in the system.  Since
ia64 uses 4GB for zone DMA (no ISA support), it's trivial to allocate a
contiguous range from the slab for bounce buffer usage.
